Accommodation Procedure for Students with a Disability

My name is Pamela Feeney and I serve as Special Services Coordinator at UMM. I work with students who require accommodations based on documented disabilities. This is the protocol for applying for ADA accommodations.

How are accommodations initiated?

In college the student is responsible for initiating the accommodations process. For a smooth transition, contact the Special Services Coordinator prior to enrollment. Provide the coordinator with documentation and diagnosis from a qualified professional.

When you arrive on campus, visit the Student Support Center to complete paperwork. Once a determination is made, relevant faculty and staff will be notified of accommodations.

It is the student’s responsibility to contact his/her instructors to activate accommodations. Students must reactivate the accommodations process each school year to continue services.

What documentation is needed and who provides it?

It is the student’s responsibility to provide documentation for a determination of services. Documentation must be certified by a qualified professional in the area of disability: medical doctor, licensed psychologist, learning specialist, or clinical counselor.

Documentation must include diagnosis of a physical, psychiatric, or learning disability.

For a learning disability, documentation must include testing results and analysis. For a physical disability, documentation must include a description of functional limitations. For a psychiatric disability, documentation must include symptoms and specific needs.

Although not required, suggestions for accommodations related to the disability are helpful.

How can I reach the Special Services Coordinator?

The Special Services Coordinator works full time for ten months from August through May. Please contact the coordinator by phone, mail, or e-mail for more information on services.
